Grades 3-12 to Participate in Student Well-Being Questionnaire
Student well-being is an important goal for our school district. Over the next several weeks (March 9-10), students in grades 3-12 will participate in a questionnaire asking them to reflect on school climate and culture as well as how they perceive their social-emotional skill sets. We are excited about continuing this process which we began last school year.
There are two versions of the questionnaire, one for grades 3-5 and one for grades 6-12. Each questionnaire contains two components: Student Supports and Environment and Student Competency Measures. Parents may view the questionnaires, as well as the message teachers will use to introduce the questionnaire to students, at the links below. While completing the questionnaire, students will be encouraged to reflect on their mindsets, approaches to learning, and feelings about school. It is our goal to help students learn more about themselves as well as to better understand ways we can alleviate barriers to learning and promote a culture of inclusive excellence.
The results from these questionnaires will allow us to better understand how Olentangy Schools can best support students’ social and emotional needs. Only administrators and school support staff will be able to view student responses from the “Student Competency Measures” portion of the questionnaire. Students’ responses for the “Student Supports & Environment” section, through which students will offer feedback about their classes, teachers, and feelings about their school, will remain anonymous. Parents/Guardians can review the questionnaires, or opt their student(s) out, using the links below.

To opt your student(s) out of the questionnaire for the 2019-2020 school year, complete the opt-out form. The deadline to opt-out for the Spring questionnaire is March 2, 2020.
