Parent's Non-Prescription Request Form



Many students come to the clinic with various aches and pains that could be relieved by Tylenol, Motrin,

Excedrin, Tums, Claritin, or another over-the-counter medication. The nurse calls the parent to

bring medication. This not only ties up a parent but may result in a student missing class until the

parent arrives, or possibly going home.


If a Parent’s Non-Prescription Medication Request Form is on file in the clinic, the student is allowed to

carry a one-day supply of over-the-counter medication in accordance with the policy as stated on the

OLSD website.



To access these forms:

· Go to Olentangy Local School District Website Go to Parents, then forms: District & Athletes,  Health Services Forms, Parents Non-Prescription Medication Request Form


· Print the form. (These are not fill-able)


· Return by one of the following methods:

· Scan and send to (underscore between first and last name)

· Fax to 740-657-5948 

· Give to your child to drop off in the clinic 

· Drop it off in the clinic yourself
